In this edition of TraumaPro we have the pleasure of looking at the work of Elijah Imlay, a writing facilitator, psychotherapist, and meditation guide. He has presented writing workshops for active duty personnel and veterans at Walter Reed Hospital and Fort Meyers in Virginia. He has also led several writing groups for veterans who live in Southern California. Elijah has attended and sometimes led the Veterans Writing Workshop organized by Maxine Hong Kingston in Petaluma, California.
As a Licensed Clinical Social Worker, Elijah has worked with women and men who suffer from posttraumatic stress disorder or related mental health issues and who are in need of healing interventions. He is on the faculty of the Institute of Applied Meditation on the Heart. A research paper entitled: "Heart Rhythm Meditation to Reduce Auditory Hallucinations and Anxiety: A Single Case Study," was published in The International Journal of Healing and Caring in September 2015. Elijah has a background in traditional methods of meditation and healing, especially those related to Sufis. These methods may help create a safe environment for the writing process.

Two Regulation Techniques
Water Breath
1. Breathe in through your nose and out through your mouth.
2. Exhale a fine stream of air, a gentle blowing and letting go, like a deep-felt sigh.
3. Inhale and imagine the crown of your head opening like a chalice.
4. As you exhale, watery energy not only passes over you like a waterfall, but it also washes within you.
5. Water, a metaphor for love, is nurturing and soothing, good for cleansing emotional wounds.
6. Let the water flow to wherever it is needed for healing.
7. Become that stream.
Hands on Heart
1. Place the palm of your right hand on your chest, then place the palm of your left hand on the right hand. Feel how comforting that is.
2. Place your attention there, about two inches above the bottom of the sternum, over the center of your heart.
3. Be aware of your breath, as if you are breathing in and out of your heart.
4. As you do this, you may feel the beating of your heart or a pulsing in your fingers or hands.
5. This is both a soothing and a grounding practice that helps reduce anxiety and prevent dissociation.
Imlay has used these methods and others at the beginning of a creative writing session with veterans of war. The Water Breath helps those who have a history of trauma to relax into a creative flow, no matter how vivid or difficult their memories may be.

An Example of Writing Therapy in the Form of a Poem
|
It is helpful for participants to give their complete attention to those who share their work out loud. By doing this, they are added to the original traumatic memories in veterans in the writing group who have been isolated by pain. This group support is usually well received. It can also be helpful on one-on-one meetings. I (Imlay) located a friend of mine 25 years after the end of the Vietnam War. He had the same nightmares every night until we reconnected. He turned those memories into prose and then I turned them into a poem out of which came healing and forgiveness. Bird Grieves for the Man They Killed

the 23rd Psalm on my helmet's

The following is a case study from Trauma, PTSD, Grief & Loss: The 10 Core Competencies for Evidence-based Treatment.
It describes how extreme trauma in children can be a more powerful cause of PTSD than actual combat.
The Relationship Between Adult PTSD
and Early Trauma
In 2011, I worked with Thom, a veteran who served 7 tours of combat in Iraq and Afghanistan. He was diagnosed with PTSD and came to see me at the urging of his wife who threatened to leave him if he did not participate in psychotherapy. He agreed to see me but was also prepared to hate me and to hate therapy. As he came into my office and we shook hands he began laughing and said that I looked like his grandfather, the only family member he liked and I thought looking like a grandfather was an interesting psychotherapy technique. He began by talking about his combat experiences and how powerful they were.
But, as I listened, I also thought there was little real evidence to suggest symptoms of intrusion, avoidance, negative alterations or arousal, the main symptom clusters for PTSD. And, although we both got a little teary at some of the events he shared, I was perplexed as to how the original diagnosis was made until he began to talk about his early childhood - especially the relationship with his stepfather.
Thom's biological father disappeared permanently when he was 3 and his mother destroyed pictures and all evidence of his father. She changed his last name to hers. When he was 7 his mother remarried and the new stepfather was a brute to this child. For example, he would frequently slap Thom on his head for no reason.
Once, when Thom was sitting and watching TV, the stepfather hit him so hard that Thom actually was lifted from the chair and flew across the room. Whenever Thom had a bad report card (which was most report cards), he was severely punished. This consisted of the stepfather having Thom sit on a chair facing the stepfather who would repeatedly hit him on his head with a spoon until Thom's head began to bleed. While the stepfather was hitting him with the spoon Thom watched the stepfather become sexually aroused. We often describe men like the stepfather as being sexual sadists. These men often marry certain women to get to their children. This sexual abuse continued until Thom was 11 and the stepfather left the family.
There is a great deal of material written about the relationship between child sexual abuse and later PTSD and we spent most of the remaining therapy sessions working on this early abuse. In particular, we used EMDR therapy which was originally developed to help clients reduce the dysregulation produced by traumatic memories. By accessing, bringing up and desensitizing these memories, the dysregulation is relieved. Thom had 14 EMDR sessions before we could terminate treatment.
