MTM-CNM Family Connection, Inc is pleased to announce that through the gifts of some generous donors, we now are able to allocate a portion of funds to provide some financial assistance to families who attend the 2017 MTM-CNM Family Conference through CONFERENCE TRAVEL GRANTS.


Eligibility & Parameters

A limited number of Conference travel grants are available to individuals affected by MTM or CNM who are participating in the 2017 MTM-CNM Family Conference. We hope that our conference travel grant program will provide support to families that may otherwise be unable to attend.

While we cannot cover all the travel expenses for families, we can provide additional support to attendees who are in a situation where they have most of their expenses covered yet need an extra boost in their travel budget. Grants will be disbursed in amounts of up to $500. 

Applications are being accepted as of now, until June 27th, 2017. If the number of applications exceeds the available funding, priority will be given based on the following considerations:
  • Those families who will have a family member diagnosed with MTM or CNM in attendance at the conference   
  • Travel distance from Nashville, TN
  • First-time MTM-CNM Family Conference attendees
  • Financial need
Timeline

Application deadline: June 27th, 2017
Notification of grants: July 3rd, 2017
If awarded a grant, receipt submission deadline: August 4th, 2017
Grant checks mailed out: August 11th, 2017

*Note that it may be helpful to draft the Personal Narrative and Financial Statements in a separate document first, and then paste them into the form when ready to submit.  Do not send your application or statements via email.

"RARE Bears" are available for our RARE MTM and CNM Warriors!
RARE Science is a non-profit organization seeking to find treatments for those affected by rare diseases, and which initiated the RARE Bear program,  a grassroots community-driven outreach for kids with rare disease. Through this program, community volunteers create one-of-a-kind teddy bears for one-of-a-kind "rare" kids. 

Erin, Marie, and Shannon were fortunate enough to connect with RARE Science at the  Global Genes RARE Advocacy Summit last September and to pick up a few of these RARE Bears!

Now, the MTM-CNM Family Connection is thrilled to partner with RARE Science to bring these bears to the 2017 MTM-CNM Family Conference and distribute them to children affected by MTM or CNM. 

In order to receive a bear at the 2017 MTM-CNM Family Conference, please download the form HERE, complete, scan, and attach it in an email to [email protected] by July 4, 2017, so we can ensure that they are shipped to the hotel by the conference date. 

***IMPORTANT INSTRUCTIONS*** Although the RARE Bear Request form provides the email for [email protected], RARE Science has asked us as collaborators in this project to collect the forms to facilitate the shipping and delivery of the bears. So please email the completed form to [email protected] to receive the bear at the conference.

For any parent(s) attending the conference without the affected child, you may still fill out a request form and bring the RARE Bear home to your child.

REGISTRATION REMINDER!!

Make sure to REGISTER for the conference if you have not already done so and are planning to attend. Remember that this step is separate from hotel reservations. 

You will receive an invoice after registering, but payment will not be due until July 7th (though we appreciate if you pay as soon as you are able). Fees go up after June 16th, so now is the time to "Get Your Boots On" and get registered!
