"Creating a Culture of Feedback at Vanderbilt"
Part 2: Definitions
In his seminal article regarding feedback in medical education published in 1983, Dr. Ende defines feedback as "information that a system uses to make adjustments in reaching a goal." 1 For a rocket engineer this might be heading, trajectory, lift, drag and speed over ground. For a learner in medicine this input will require data about the 6 domains of competence (Medical Knowledge, Patient Care, Professionalism, Practice Based Learning and Improvement, Interpersonal Communication Skills, and Systems Based Practice). Input about a learner's current level of function relative to the ultimate goal will help inform their areas of focus for improvement.
For many of us, feedback specifically brings to mind the oral transmission of data about a learner. In fact, this face to face interaction is a vital component of the feedback process because it allows for the learner to engage in a conversation to explore areas for improvement and specific items that need attention. But this conversation is not the only element that comprises "feedback". Written assessments and evaluations are often used in education and are considered feedback. For some educators, the term assessment is used only for the performance of people and evaluation is used only for the performance of a curriculum or program. Others do not make this differentiation, but instead view assessment as formative (guiding) feedback and evaluation as summative (final) feedback. For some, assessment focuses on the process of learning and evaluation on the product of learning.
As we move further through this series on feedback we will begin to explore how to gather data about a learner, compare their progress to a reference standard, deliver both formative and summative data and create a plan for change. This will include how to observe the learner for specific behaviors using a reference standard such as the "milestones" now prevalent in all graduate medical education programs and many undergraduate medical education programs.

Rebecca Swan, M.D.

Current Teaching/Educator Activities:
Dr. Rebecca Swan spends much of her time as the Program Director of Pediatric Residency, with direct responsibility for 75 Categorical Pediatric residents as well as working with 16 Internal Medicine-Pediatric residents. As a clinician educator, she values her time in the General Pediatric Primary Care Clinic, precepting 4 residents in their continuity clinics on a weekly basis. Dr. Swan has been collaborating with peers through the AAMC around professional identity formation. She was involved with a workshop at the AAMC Education Meeting in November and another one will be presented at the spring SGEA Meeting.
Educator Project Summary:
This fall, Dr. Swan was delighted to work with colleagues to develop and host an ACGME- VUMC Faculty Development Course: "Clinical Evaluation and Assessment of Residents and Fellows". She presented two lectures, "Medical Educator Competencies" and "Educator Competencies" and served as one of the facilitators for "Feedback on Your Feedback" workshop. This pilot course was attended by a diverse group of educators from the southeast and beyond, and was well-received, with plans to replicate it in the future.

Passion for Medical Education:
In addition to the daily administration of a large residency program, Dr. Swan's areas of focus are in professionalism, mentoring, feedback and communication, resident wellness, and increasing diversity in graduate medical education. Having served as program director since 2002, she has seen changes impacting graduate medical education from many directions: the ACGME, health care industry, the public and patients, scientific discoveries as well as generational changes in the learners. These changes have challenged Dr. Swan in her career to broaden her knowledge and professional contacts, to think differently about how we educate, how we assess and how we mentor; and to consider the impact of how we guide learners through their education trajectory towards early careers. These challenges continue to fuel Dr. Swan's passion for GME and are the basis for her receipt of the 2012 Parker J. Palmer Courage to Teach award from the ACGME for innovation!
