NEW YEAR AND NEW TECHNOLOGY

On January 10, 2023, KCHC/Pillar Health launched Epic, a new electronic health record (EHR), that includes a new patient portal called MyChart. There are many benefits to MyChart. It will allow you to send a message to your provider, pay for services, and have better access to your health information. During your next appointment, ask our team about getting enrolled in MyChart.


CareMessage, our popular messaging program, has been updated as well. Please stay in the program to continue receiving automated appointment reminders and health tips.

 

We continue to ask for patience as the KCHC/Pillar Health Team works to provide you with expert services while simultaneously learning how to use the new EHR. Some delays are inevitable, but you can be assured that these are only temporary and will be resolved soon.

JANUARY IS CERVICAL CANCER AWARENESS MONTH

According to the CDC, each year in the United States, about 13,000 new cases of cervical cancer are diagnosed and about 4,000 women die of this cancer. Hispanic women have the highest rates of developing cervical cancer, and Black women have the highest rates of dying from cervical cancer. Cervical cancer is preventable thanks to the human papillomavirus (HPV) vaccine

NURSE CARE COORDINATION PROGRAM ANNOUNCEMENT

KCHC/Pillar Health has implemented a Nurse Care Coordination Program. In fact, three (3) experienced Registered Nurses (RNs) have been added to the team to continue building and improving the services of the program. Nurse Care Coordination utilizes the unique skills of nurses to manage and coordinate the care of patients and strategically positions RNs at different points of care to address inequities in healthcare.

 

The RNs will take the lead in providing care in three essential areas:

  • Patient intake where clients will be evaluated for medical, behavioral, and oral health needs, as well as social determinants of health assessments, and referral to community-based resources.
  • Chronic care management focused on patients with two or more chronic medical conditions who have the greatest needs and challenges in managing their health; and
  • The transition of care coordination for KCHC and potential new patients who recently used the emergency department or were hospitalized and need appropriate follow-up provider visits and monitoring of their progress following discharge.

 

The Nurse Care Coordination Program has been designed to help patients navigate through the complex medical system.

THE CHILDREN'S DENTAL MONTH COLORING CONTEST RETURNS THIS FEBRUARY

February is National Children’s Dental Health Month. Help Kenosha Community Health Center and Pillar Health celebrate and elevate the importance of healthy oral hygiene in kids by taking part in our Coloring Contest!

The contest is open to children ages 4-11 who reside in Kenosha, Racine, Walworth, and Milwaukee counties.
