Nurse Care Manager/Care Coordinator Standardized Core Curriculum (GLearn) Training Program
CTC-RI, with funding from UnitedHealthcare, is pleased to offer up to ten Nurse Care Managers/Care Coordinators (NCM/CC) the opportunity to participate in a standardized evidence-based training program on key care management concepts within primary care.

Delivering Age-Friendly Care: Implementing the 4M Model and Supporting Care Partners of People with Dementia
CTC-RI, in collaboration with the RI Dept. of Health and UnitedHealthcare, is offering $6,000 in funding to six primary care practices/organizations to participate in a six-month quality improvement initiative using the Age-Friendly Health System 4M framework to improve care for older adults and people living with dementia.

Dr. Michael Koster shares evidence-based guidelines, advice on resurgence of preventable diseases
At CTC-RI’s July Improving Child Health in RI meeting, Dr. Michael Koster shared insights on the resurgence of measles, varicella, and pertussis, emphasizing the importance of vaccine confidence and communication strategies for providers.

Adult Immunization Quality Improvement Learning Collab. wraps up, shows improved rates
Seven primary care practices presented their results at the final session of the CTC-RI Adult Immunization QI Learning Collaborative, demonstrating improved immunization rates for COVID-19, pneumococcal, influenza, and Tdap, particularly among high-risk populations, including pregnant people and individuals with diabetes.

CTC-RI Launches Autism Early Identification Initiative
With support from UnitedHealthcare and the Rhode Island Department of Health, CTC-RI launched a new initiative to equip pediatric and family medicine providers with training in early autism and developmental delay identification. Fifteen providers, with training led by Brown University, Hasbro Children’s Hospital, and Vanderbilt University experts, will work to improve early diagnosis efforts and connect children to services sooner.

RI Practices Complete STI Quality Improvement Initiative
Nine primary care practices across Rhode Island recently wrapped up a quality improvement initiative aimed at strengthening Sexually Transmitted Infection (STI) screening and treatment. With STI rates on the rise, this work is helping to ensure earlier detection, prompt treatment, and better patient outcomes.

Charting Rhode Island's Primary Care Future
In a recent session, CTC-RI’s Clinical Strategy Committee, joined by national expert Chris Koller, explored how Rhode Island can lead in primary care transformation, tackling issues like workforce shortages, payment models, and shared responsibility for whole-person care. Key priorities emerged to help strengthen advocacy, funding, and accountability.

Registration is open for our Annual Conference on November 19! Join CTC-RI in Warwick for a day of dynamic learning, networking, and collaboration. Attendees will engage with timely topics including health equity, behavioral health integration, workforce challenges, population health, and technology—all aimed at strengthening Rhode Island’s primary care system.
