🩸 It's High Blood Pressure Education Month––

Evidence-Based Lifestyle Change Health Programs Work!

CTC-RI recognizes May as High Blood Pressure Education Month and supports efforts to reduce hypertension, which affects one in three adults in Rhode Island.

The Rhode Island Department of Health's Healthy Heart Ambassador Blood Pressure Self-Monitoring program is a free, evidence-based resource helping Rhode Islanders manage high blood pressure through self-monitoring and nutrition support. The program is offered through several local organizations, including the Community Health Network––a one stop shop, for evidence-based lifestyle change health programs, with many impactful resources for primary care patients.

🔎 Spotlight: RI's Cardiovascular Disease Learning Collaborative

In recognition of High Blood Pressure Education Month, CTC-RI connected with Breanne DeWolf, Health Systems Transformation Manager at the Rhode Island Department of Health (RIDOH), and Jayne Daylor, Quality Improvement Consultant for CTC-RI/RIDOH. Together, they share insight into the launch, goals, and early lessons from the Rhode Island Cardiovascular Disease Learning Collaborative, an initiative designed to strengthen chronic disease prevention and management among Rhode Island’s free clinics.

"The Rhode Island primary care community faces many challenges, for this learning collaborative, currently, we are exclusively working with Rhode Island’s only two free clinics that support some of the most vulnerable uninsured Rhode Islanders."
