MAPP 2.0 Update: A Call for Nonprofits to Complete the Community Partner Assessment

Mobilizing for Action through Planning and Partnerships (MAPP) is a community-driven strategic planning process to achieve health equity. MAPP provides a structure for communities to assess their most pressing population health issues and align resources across sectors for strategic action. It emphasizes the vital role of broad stakeholders and community engagement, the need for policy, systems and environmental change and alignment of community resources toward shared goals. This process results in the creation of a Community Health Assessment (CHA) and a Community Health Improvement Plan (CHIP) for Lake County. Click HERE to learn more about the MAPP process.

Thank you to all of the organizations who have completed the Community Partner Assessment! We have had a great response!

We wanted to put out one final reminder – for any of our Live Well Lake County organizations who have not yet been able to complete the Community Partner Assessment. If your organization has already completed it, you may disregard this final reminder.

For everyone else, as a reminder, we would like your organization to participate in our Community Partner Assessment. This survey will help develop a collective assessment of organizations that are working to improve health and well-being in Lake County.

The Link to the Community Partner Assessment is here:

Community Partner Assessment

Here are some quick notes regarding the Community Partner Assessment:

  • Please have only 1 person per organization complete the survey.
  • NOTE #1 about organizations: If you and your organization are sharing responsibilities for completing the assessment, some partners have found that having the list of questions in the entire survey, to share with teammates, is helpful. Please find attached in the May 28 reminder the list of questions from the Community Partner Assessment.
  • NOTE #2 about individual partners within the LWLC Network: If you are a member of the LWLC Network, but you aren’t from an organization, please don’t worry about this assessment. There will be other assessments in the coming months where your voice and perspective will be heard and included.
  • The survey takes approximately 30-45 minutes to complete. Thank you in advance for your time with this.
  • With this extension of the assessment, please make sure your organization’s assessment is completed and submitted by this Friday, June 6th.
  • For more information or for assistance in completing the survey, please feel free to contact Christina Hayes at CHayes2@lakecountyil.gov.
