ARRIVAL: Please arrive at DESIGNATED MEETING SPOT based on times below for each project. If you are dropping off a volunteer, they must be signed in and out at our SmileUp! table by the same person (must be over the age of 18).

WAIVER: The following is the verbiage that must be signed off on for each participant once you arrive onsite. If you can not agree to the verbiage, please cancel your registration.

In addition to any other risks posed by participating or volunteering with Smile Up Charitable Foundation (Organization), the individual signing below understands that, despite any safety precautions being taken by Organization, by participating or volunteering with Organization, there is a risk of potential exposure to COVID-19 or any other harmful virus or bacteria, which may result in illness or death. The individual signing below releases, forever discharges, indemnifies and holds harmless Organization and its directors, officers, employees, volunteers, successors and assigns from any and all liability, claim, costs or expense related to such risk.
