We are approximately two months away from the close out of ARPA and we are excited to continue sharing updates of all the hard work that ARPA project teams have been working on and the results that they have produced. We are working on new material to share publicly all of the incredible outcomes of this work in order to recognize the long lasting impact that this funding is and will continue to have on the Colorado HCBS system and those who benefit from it. We look forward to sharing that with you this year.

Translation of Case Management Material |
This project successfully translated 116 documents (63 unique source documents) in up to six languages totaling over 142,000 words translated. These documents include Member Letters, Forms, Applications, Guides, Charts, Fact Sheets, Brochures, Social Media Messaging, and more. The audience for these documents includes case managers, members, direct care workers, stakeholders, and providers.

Expand the Behavioral Health Safety Net |
This project supported significant behavioral health safety net reforms by developing improved standards and funding rates for providers, developing mechanisms for ongoing support, training, and technical assistance for new and existing providers, and expanding a critical level of services to help individuals achieve treatment success in the most appropriate level of care.

Child/Youth Step-down Options Program and Provider Recruitment | |
This project developed three grant programs designed to support the child/youth population. Grants focused on expanding capacity and conducting research on barriers and gaps in accessing services. The project also researched the financial impact of expanding the Medicaid Expansion Children’s Health Insurance Program (CHIP) program (M-CHIP) program.

A total of $2,936,345.00 was provided through grant awards to two organizations who supported the needs of 368 Members, ages 18 and older. These organizations used the grant funds to establish, promote and expand CAPABLE services in ten counties in Colorado.
Community Aging in Place—Advancing Better Living for Elders (CAPABLE) is a person-directed, home-based program that addresses both function and healthcare expenses. The four to five-month program integrates services from an occupational therapist (OT), a registered nurse (RN), and a handy worker who work together with the older adult to set goals and direct action plans that change behaviors to improve health, independence, and safety.

Case/Care Management Best Practices | |
This project researched national best practices for Colorado’s Regional Accountable Entity (RAE) / Case Management Agency (CMA) system to better meet the needs of a variety of member profiles such as complex care coordination for those with dual or poly diagnoses. Training plans were developed, which included appropriate materials for CMA and RAE staff on their various roles and responsibilities, collaborative roles between the systems, and effective collaboration across the continuum of care, especially for members with complex needs. Finally, funding was provided to CMAs to incentivize the implementation of identified best practices.

Member-Facing Provider Finder Tool Improvement |
This project added enhanced searching features to the existing "Find a Doctor" tool on the Health First Colorado website as well as the PEAK member portal. Enhancements included the ability to filter by services provided in a specific county, new fields for alternate provider telephone numbers and/or addresses, provider cultural competencies, provider membership to a community association, provider language proficiencies, and providers’ preferred name.

This goal of this project was to provide training and educational materials for providers and members in emergency preparedness. 80 course opportunities were offered and a learning management system (LMS) was developed to house virtual training options that remain available to members and providers free of charge.

Member Emergency Preparedness | |
This project focused on directly supporting members to be prepared for an emergency. In collaboration with the University of Colorado (CU) Center for Inclusive Design and Engineering (CIDE), the project distributed 1,000 disability specific Go-Kits as well as 1,000+ battery-powered back-up power devices to members across the state.

Pay for Performance for PACE |
This project created a Value Based Payment (VBP) program for the Programs for All Inclusive Care for the Elderly (PACE). As part of the development process, relevant best practices and current measurement processes were reviewed in an effort to create alignment where possible and promote opportunities for improving quality through potential payment incentives.
