Project Metrics around HIT are focused on the work of the Statewide HIT Task force and we are currently investigating state and regional approaches to address some  HIT needs including shared care plans. All project metrics and descriptions can be found in the Region 7 IDN Project Plan.

June 1- Critical Time Intervention Kick Off- Hosted by IDN Regions 1, 3, 4, 6 & 7!  Daniel Herman, PhD, Director of the Center for the Advancement of CTI, will guide our learning process. Dr. Herman has led CTI research & dissemination for over 15 years. Regions will share CTI implementation plans. Lunch will be provided.

Clinical Work Group
  • Integrated Healthcare is the Hub of all projects;
  • Work group is going step by step through "B1" Integrated Health project, focused on the key components and requirements for a Multi-Disciplinary Core Team;
  • Determining what assessment and screening tools are used and what is needed;
  • Looking at training requirements for all staff and what is currently utilized and required;
  • Gathering position descriptions for care coordinating positions.
 
Data Work Group
Bruce Blaney from DHHS (Department of Health and Human Services) presented on the WITS (Web Information Technology) state reporting system. WITS is an EHR (Electronic Health Record).
 
Community Work Group
  • Review of current communication materials; this newsletter, Basecamp and website www.region7IDN.org., Facebook etc.
  • Who do we reach out to, how, and is it time?
  • Developed an elevator speech which will be forwarded to the steering committee for review.
Finance Work Group did not meet 
KEY CONVERSATIONS
The Consortium has been meeting with Region 7 partners. Some partners had questions regarding the upcoming funding proposals due 3PM May 15.
Key Points:
  • Your concept paper must address the deliverables in the original application made to the state on behalf of Region 7.
  • Your concept paper must include the deliverables outlined in the project metrics provided by the state.
  • Favorable projects will show collaboration and leverage existing strengths/programs and community supports and services.
  • This is a "Demonstration" project which provides an opportunity to demonstrate new ideas and different approaches.
