HEALTHY KIDS COLORADO SURVEY
Parents,

Please review the information about Healthy Kids Colorado Survey that will take place on November 9th. Opt out form is below if you do not want to take the survey.
COME GET YOUR FREE T-SHIRT or DECAL if you take the the survey.

The purpose of this survey is to help the school identify immediate issues that may exist or need to be addressed to provide a more effective and positive school environment for your student. Your answers in the survey will be anonymous and not identify you or your student. The information obtained from answering the survey questions will help the school develop comprehensive school counseling services offered to your student that will help increase the graduation rates and better prepare students for postsecondary opportunities and success after graduation.

The School Counselor Corp Grant program has helped 365 secondary schools in 98 of the 178 districts and is now helping to create a more comprehensive school counseling program that will benefit your student. Needs assessments provide many advantages to school counselors, teachers, and administrators by monitoring student progress to close achievement gaps, assess and evaluate programs and help develop programs that align with the school and district mission. They can address and identify any deficiencies in current school services and programs, and address those areas where school counselors can respond with direct action. A needs assessment could also identify demographics and social groups within a school that may be in need of intervention or nurturing. Needs assessments can also be used as part of larger program evaluations to determine their effectiveness and identify ways in which student populations can be better served.

Florence Junior Senior High wishes to thank you for your time and effort in helping us meet the needs of your student by completing this survey.
