To: All Support Gainful Employment QIA Facility Staff, Patient SMES and Stakeholders
March is Innovation month
We are off to a strong start for this QIA, and the challenge to you and your staff for March is to be INNOVATIVE and "think outside the box"  in promoting patient engagement and vocational rehabilitation (VR).
 
The Network's  March Innovation:
Streamlining for less paperwork :  We are partnering with the Patient Services Department to include their Monthly Patient Engagement facility assessment in the Monthly Status Report to reduce burden and superfluous data collection.
 
Please review the March MSR worksheet (PDF)  to see how you are being asked to report on March activities, and submit your answers online by March 31st.

Please utilize this checklist with at least 2 patients in March while developing annual plan of care patient meetings.  
 
MEI Patient Checklist Reporting:
Please discuss the topics of interest that patients requested information on in your March QAPI meeting, and on the MSR, report the number of patients utilizing the form and the resulting requests for more information on TREATMENT Work/School/Insurance section only.  Also, provide feedback (Adopt/Adapt/Abandon) on the usefulness of form, and any patient comments about form.

Visual Display/Education Station for your QIA(s)
How is your Education Station Progressing?
Are you working with your staff and patients to develop an Education Station?  Are there Barriers or Successes? March is the month to review your progress - what stage are you at? (Planning, Launching, or Procrastinating?)
 
Pro Tip: Remember to ask for suggestions and integrate any feedback or innovation. This can be a bulletin board, a table display or something outside of the box (INNOVATION Is the key to success!)

Make sure you have a section that says - if you are interested in going back to work or school, speak with the staff.  (hint: the SSA's Ticket To Work Program - qualifies as both a VR referral and can count as utilization if they make the call or sign up for an online webinar)

Reporting: By June 2019, you will be asked to submit a picture of your education station to the Network for our Annual Education Station Contest.

Patient Knowledge Assessment
Assess at least 10% of your patients on their knowledge about your QIA topic.  This is a brief assessment that can be completed verbally and/or in written format. The baseline questions are:

Did you know that the government has work/school programs for people with disabilities?  

Did you know that being on dialysis may qualify you for these programs?

Reporting: Please discuss the results of this assessment in your March QAPI meeting, and in March, you will be asked to provide the number of patients assessed (at least 10% of your census), the answer totals, and any patient comments about the questions in your monthly status report. 
Project Goals
Support Gainful Employment/Vocational Rehabilitation QIA : Participating facilities shall demonstrate a 10% increase  in VR Referrals (reported in CROWNWeb) and a 5% increase in VR Utilization (reported in CROWNWeb)  by September 30, 2019.
