Forms 1099 Mailing
QualChoice will mail all IRS Forms 1099 by January 31. If you did not update your W-9 in 2019, please fax a completed, signed and dated IRS Form W-9 to the QualChoice Finance Department at 501.707.6728.

Revised IRS Forms W-9 must be received by QualChoice no later than the end of February, 2020. For questions, please contact Finance_Ops@qualchoice.com .
Reminder: Download Supplemental Tax Reporting Data
For help in completing their tax forms, employers can download a spreadsheet of enrollment data from their My Account home page at QualChoice.com. We encourage them to review the information carefully and let us know of any changes or corrections needed. For questions or concerns, they should contact their Account Service Representative .

Commission Payment Change
You may have noticed on your bank statement that your commission payments are now paid by Centene Corporation in St. Louis. The commission and payment processes remain the same as always, and all communications will continue to come from QualChoice. 
Flu Shot Reminder
The Arkansas Department of Health reports widespread flu activity in the state. As of January 4, there had been 15 flu-related deaths so far this season. Remind clients that QualChoice members can get the vaccine with no out-of-pocket cost. Learn more and find a vaccine provider .
Holiday Closure
