Inside the Google form are three pages of release forms to download, print, fill out, and sign. These forms can be scanned and uploaded to the Google Form, or they can be sent back with your student. If you do not have access to a printer, please ask your student to bring home the printed release forms from school.
It is possible to fill out the Google form without uploading your release forms, so whether you can upload them or not, please fill out and SUBMIT the form right away!
