Verification of Public Assistance
Current total monthly gross household income (before taxes)
All l income for every adult over 18 years of age
Provide proof of negative impact
Photos of home damage
Any receipts of expenses related to home repair, or hotel cost
Verify City/County residence, verify 2 TWO of the following items:
Valid Texas driver's license or identification card with name and Harris County address (ex. Matricula Consular, etc.) and Harris county address.
current utility bill with applicant’s name and address
Proof of public benefits documentation for 2 TWO of any of the following programs
CURRENT statement: Medicaid, SNAP, WIC, CHIP, Social Security, Disability, Harris County Gold Card
Verify at least 2 TWO documents corresponding to negative impact
2019 or 2020 tax return; or
2020 W2(s) for every job held by each adult household member;
Employer letter verifying fewer hours/decreased wages or loss of income/wages
Unemployment compensation statement showing 2020 income;
Paystubs for the last 30 days; or
Complete Connective self-employment form
