Forms 1099 Mailing

IRS Forms 1099 will be mailed by January 31, 2023. If you did not update your W-9 in 2022, please fax a completed, signed and dated IRS Form W-9 to the QualChoice Finance Department at 501.707.6728.


Revised IRS Forms W-9 must be received by QualChoice no later than the end of February, 2023.

For help in completing their tax forms, employers can download a spreadsheet of enrollment data from their My Account home page at QualChoice.com. We encourage them to review the information carefully and let us know of any changes or corrections needed. For questions or concerns, they should contact their Account Service Representative.

Form 1094-C/1095-C Deadlines

For the 2022 reporting cycle, applicable large employers (ALEs)employers with 50 or more full-time or equivalent employeesmust furnish individual statements to full-time employees no later than March 2, 2023.


ALEs must file applicable reports with IRS by no later than Feb. 28, 2023, if filing by paper, or no later than March 31, 2023 if filing electronically (which is mandatory for ALEs filing more than 250 forms).

Reminder: Life Benefits Carrier Change

In January 2022, QualChoice partnered with Sun Life for the life benefits offered to our group business. We have realized that this transition was not ideal, and we understand the importance of keeping things simple. That’s why, effective January 2023, we are offering our QualChoice life coverage to all groups. Any existing groups will also transition to our QualChoice life coverage when they renew in 2023.

Maternal Health Awareness Day

Since 2021, the American College of Obstetrics and Gynecology recognizes Maternal Health Awareness Day on January 23rd to raise awareness of maternal health issues and risks. According to the CDC, over 80% of maternal deaths in the United States are preventable.


The QCARE Maternity Management program is available to participating plans to help members enjoy a healthy pregnancy and deliver a healthy newborn. A Special Additions nurse acts as an advisor and guide, coaching the member and sharing information.


Our maternity care manager will:

  • Review the member's health history, diet and exercise
  • Spot and assess potential risks
  • Suggest ways to lessen risks for mother and baby
  • Share information on pregnancy, childcare, labor and delivery
  • Work with the doctor to coordinate care
  • Point the member toward community resources
