Good afternoon, community leaders!

As promised, we are following up on last week's E-Blast with more information about the HMIS Security Audit. Please follow these steps you need to take to successfully complete your audit:

1. Designate someone at your organization who will be responsible for reviewing your organization's security compliance and completing the HMIS Security Audit form. Ideally, your designated Security Officer will be the person to fill out this form. We only need one audit form submission per organization.
2. Do you have the Consumer Notice posted in your lobby? Is your anti-virus software up to date? Are all physical files secure? Use the checklist to review all of the audit requirements and complete all spot checks so you are ready to answer all of the audit questions.
3. When you are ready, complete and submit the online HMIS Security Audit Form. Note: You must complete the full audit form in one sitting as there are no save options in this online form.
4. Once we receive your submission, our staff will review it and reach out to you to let you know if it is accepted or if we need additional information from you.
