Dear Parent/Guardian:
Your child has medication in the school health office. Medication needs to be picked up by the last day of school. Medication that is not picked up will be packaged, sealed, and sent home in your child's backpack on the last day of school.
Please note, however, that controlled medications WILL NOT be sent home with students (methylphenidate, ritalin, adderall, diazepam, etc..). If you are unable to pick up medication and need to make a special arrangement with the school, call the health office. Otherwise, medication that is not picked up by the last school day will be disposed of.
Medications will NOT be kept in the health office over the summer, transported to summer school, or stored for the following school year.
2025-26 School Year
Attached you will find a medication authorization form. This form needs to be completed every school year. The form has been provided to you now so there is time to complete it during summer break and return it on the first day of school.
Prescription medication will not be given without a written provider’s order and parent permission. The medication must be in the original bottle.
For a student who takes medication such as stimulant medication for ADHD, we ask that you do one of the following when sending a new supply of medication to school:
- An adult hand-carry the medication to the school health office
--or--
- An adult will call the health office at the student’s school and report the amount of medication will arrive at school that day with their child.
