- Are you interested in advising AFHTO on the top issues facing team-based primary care in Ontario?
- Would you like to be a leader at AFHTO to advocate for and to speak to the critical role of teams?
- Would you like to share and spread knowledge and learnings?

Now more than ever, it is a critical time to engage and collaborate with leadership across AFHTO members to:
- Inform advocacy efforts that AFHTO undertakes on behalf of its members
- Provide advice and input to AFHTO Board and staff on issues that matter to AFHTO members (i.e. advocacy, operations, governance and programmatic opportunities)
- Advance best practices through knowledge transfer and exchange across Boards, Executive Directors and Lead providers and member organizations
- Demonstrate and advance leadership to assist primary care teams to achieve effective and efficient team work and care delivery
We are specifically recruiting representation from the following LHIN regions:
LHIN 1,4 and 5, OH central, OH east and Francophone representation.
We invite interested EDs/Admin leads, board chairs, lead physicians or NP leads to apply by completing the application form (2025 Application for AFHTO's Leadership Council Survey) by, January 31, 2025.
