211CT HealthyLives Navigator

There is a growing awareness  that social and economic factors have a significant impact on an individual's ability to lead a healthy life.  2-1-1 Connecticut's new HealthyLives Navigator is an  online tool designed to screen for social determinants of health.  The HealthyLives Navigator is a  brief, anonymous questionnaire that can be completed by clients, healthcare providers, case managers, or social workers. It assesses for the eleven (11) social needs that are most closely linked with poor health outcomes.

Access to nutritious food, stable housing, reliable transportation, substance use treatment and mental health services greatly influence a person's health outcomes. The 211CT HealthyLives Navigator can help identify needs that you or those you care for may have - and help connect you to resources to meet those needs.

Holiday Meals and Resources
 
There are a number of organizations throughout the state working to make the holidays a happy time for families in need throughout Connecticut. Whether food, gifts, or holiday meals, 2-1-1 has the most up-to-date information about the holiday assistance available. Visit  211ct.org or call 2-1-1 for information on resources and services being offered across the state.

The 2-1-1 website currently lists more than 60 locations for meals this Thanksgiving. Click HERE or visit  www.211ct.org and search, "Thanksgiving Meals" to find information on Thanksgiving meals being offered in your area.

About the Go CT Card
The Go CT Card is a new "tap and go" fare card that allows p ublic transit commuters who ride the state's bus system to conveniently tap their Go CT Card on the fare box when boarding. Riders who use the card will automatically receive any discounts that are available, providing them with the lowest possible fare.

The Go CT Card, is now available to use on all CTtransit, CTtra nsit Express, and CTfastrak buses throughout the state. Get your Go CT Card for just $2 at  www.goctcard.com.

After receiving the card, riders can add transit travel funds into the account in one of two ways - funds can be added online at www.goctcard.com, or in person by visiting a participating retailer (CVS Pharmacy and 7-Eleven) or one of the CTtransit Customer Service Centers.

Connecticut 2-1-1 maintains a database that includes nearly 500 support groups focused on a variety of needs and life circumstances including: 
  • Parent Support Groups
  • Domestic Violence Support Groups
  • Substance Use Support Groups
  • Support Groups for Loss of Loved Ones
  • Alzeheimer's Support Groups
  • Kinship Caregiver Support Groups
  • Any many more
S upport groups create an environment where individuals can share their experiences, strengths, and hopes with others.
