Hello 4th Grade Parents!
It’s time for Carnival and the DUNK BOOTH on Friday, October 24th! If your student plans to participate in the Dunk Booth, parents must submit this waiver by Wednesday, October 22nd. Hard copy waivers went home in your student's Friday folder, or you can complete it online using the link below.
DUNK TANK CONSENT AND WAIVER
Those with signed waivers will receive this fun Dunk Tank T-Shirt on Wednesday, October 22nd. This will be your child’s ticket to access the Dunk Booth, so please have them wear their shirts to school on Friday, October 24th!!
