The United Center mass vaccination site at 1724 W. Madison opens March 9.

The vaccination site is by appointment only. Parking is on site and a drive thru option will be available later in the month.

Seniors 65+ will have an early registration period from Thursday, March 4 at 8:30 am through Sunday, March 7 at 4:00 pm. Go to Zocdoc.com/vaccine to register for an appointment.

Registration for Illinoisans who meet the state’s Phase 1b+ eligibility criteria will open Sunday, March 7 at 4:00 pm.

There is NO ONSITE REGISTRATION. You must have an appointment. Go to Zocdoc.com/vaccine to register for an appointment.

If you cannot access the website, call 312-746-4835. This hotline is open Monday – Saturday 8:00 am – 8:00 pm and Sunday 8:00am – 4:00pm. (On Thursday, March 4 the hotline will open at 8:30 am.)

The Federally-run site will be able to administer up to 6,000 doses per day in Chicago. This is ADDITIONAL vaccine and is not taken out of the City’s allotment.

How to get to the United Center:

Driving: Entrance to the vaccine site is in Lot E of the United Center, off of Madison St. Parking is available in Lot A with a free shuttle to the vaccination site.

UBER: Rides credits from Uber

Uber is providing $40 ride credits to Chicagoans. After a Chicago resident books an appointment via Zocdoc, they will receive an appointment confirmation email which will also ask if they need transportation to their visit. If they answer yes, they’ll receive ride credits from Uber which will work only for rides to and from the United Center within the Chicago city limits.

  • Your first contact should be your primary care provider, health clinic, or hospital where you have gotten medical care in the past.
  • Health care providers can still sign-up to be vaccine providers.

Zocdoc

  • Zocdoc's new COVID-19 website is designed to help residents book appointments to receive the vaccine at City POD sites and community healthcare organizations. Residents will need to first confirm their eligibility and complete a brief pre-screening questionnaire.
  • While all available appointments are currently booked, after completing the pre-screening questionnaire, you can sign up to receive email notifications letting you know when appointments become available.

Your Pharmacy
The City has provided vaccines to pharmacies across Chicago. These pharmacies each have their own registration link, which can be found below. 


Cook County

Cook County has several mass vaccination sites and Chicagoans can access them. You must sign up at https://vaccine.cookcountyil.gov.
