three-phase rollout for the COVID-19 vaccine.

Here at Monadnock Community Hospital, we are committed to keeping you informed of COVID vaccine news -and helping you get vaccinated as soon as possible.
Those currently eligible to receive the vaccine meet the Phase1b criteria below:
1) Those 65 years and older
2) Those under 65 and have 2 or more underlying medical conditions listed below (adapted from CDC)
- Cancer
- Chronic Kidney Disease
- COPD (Chronic Obstructive Pulmonary Disease) and other high-risk pulmonary disease
- Down Syndrome
- Heart Conditions, such as heart failure or coronary artery disease
- Immunocompromised states
- Obesity (body mass index of 30 kg/m or higher)
- Pregnancy
- Sickle cell disease
- Type 2 Diabetes Mellitus

Q. If I'm between 16-64 and qualify for the vaccine, now what?
A. If you're a patient of MCH and believe you qualify with two underlying conditions, but have not received an email from your PCP, please call your PCP's office to confirm We've already notified the State of New Hampshire of all of our patients who qualify to receive a vaccine in Phase 1b. The State is currently contacting those people by email to let them know they can log into the State's website to register! If you ARE NOT a patient of MCH you may also be vaccinated at the State-run vaccination sites, with confirmation by your physician that you have 2 or more high-risk health conditions. NOTE: The CDC's Vaccine Administration System will send you an email from: no-reply@mail.vams.cdc.gov. Note: Don't forget to check your junk mailbox. Some people report finding this email notification went there.
Q. Once I qualify, where can I get my vaccine?
A. Across the state, local Public Health Networks are supporting the vaccine distribution by increasing capacity through the administration of State-run vaccination clinics that have been established to help make vaccines available through the quickest and most efficient distribution process possible. NH Clinic locations currently include: Keene (Greater Monadnock), Concord (Capital Area), Nashua (Greater Nashua), Claremont (Sullivan), Exeter (Seacoast), Hooksett (Greater Manchester), Laconia (Winnipesaukee), Tamworth (Carroll County), Littleton (North Country), Londonderry (South Central), Plymouth (Central NH) and Rochester (Strafford County).
Locally, the Greater Monadnock Public Health Network serves 33 towns in the Monadnock Region and is supporting regional vaccines through the Keene vaccine clinic location with the leadership of the National Guard. MCH and Cheshire Medical Center are supporting the Public Health Network’s effort by providing information about the distribution, communicating vaccine clinic details, supporting community clinic volunteers as well as working with the Public Health Network to establish a process to support any homebound patients who would be unable to travel to a vaccine clinic.
Q. I don’t have transportation. Who can help me get to my vaccination appointments?"
A. Local volunteer resources to support those without access to transportation for their COVID-19 vaccine appointment:
Phone: 603-821-0569. CVTC also provides rides for people whose mobility is dependent on a wheelchair. CVTC covers the full Greater Monadnock Public Health Region (map included on the website)
- CVTC will cover weekday and weekend appointments
- 1 rider at a time, or two household members (rides happen in personal vehicles, not buses/shuttles)

- Rides only during the week 8am – 4pm (schedule availability permitting of course)
Q. Can I bring my spouse or a family member to my vaccine appointment?
A. Yes, you can come with your spouse or family member, however they may only receive the vaccine if they have an appointment. When registering online on the NH Vaccine website, there is an option to indicate that you will be arriving at your appointment with another Phase 1B eligible member who would like to be vaccinated. That person will verify and complete registration paperwork onsite. .
Q. I found available dates at one of the Clinics, but it's not very near my home. Should I wait to see if more dates are added to a location closer to my home?
A. No. We strongly advise you to keep the earliest appointment you're able to schedule and plan to travel to that location. As more eligible groups of NH residents are allowed to register, wait times for available appointments are expected to grow. You can receive your vaccine at any of the 13 State-run locations listed above.
