CONNECTING GEORGIA:

MOMENTUM, INNOVATION & IMPACT

A lot is happening across the GaHIN network—and even more to be excited about. From new investments in rural healthcare to expanded behavioral health initiatives and technology enhancements, GaHIN continues to strengthen the connections that make coordinated, whole-person care possible across Georgia.

HIGHLIGHT: GaHIN Selected for Phase 2 GREAT Health Funding

More investment. More collaboration. More opportunities to strengthen healthcare across Georgia.

GaHIN is honored to be selected as a Phase 2 awardee under Initiative 2: Strengthening the Continuum of Care in Rural Georgia, part of the Georgia Department of Community Health's GREAT Health Program.

A total of $4.6 million was awarded to grantees under Initiative 2, which supports projects designed to improve care coordination and strengthen connections among healthcare, behavioral health, public health, and education partners. GaHIN joins Morehouse School of Medicine, Georgia State University, the Georgia Department of Behavioral Health and Developmental Disabilities (DBHDD), and the Georgia Department of Education as awardees.

For GaHIN, this recognition reinforces the importance of the work we are doing alongside our members and partners to create a more connected healthcare ecosystem—particularly for Georgia's rural communities.

WHAT WE'RE WORKING ON

GaHIN continues to move beyond simply exchanging information. Our work is increasingly focused on making information actionable—connecting clinical care, behavioral health and social services so teams can coordinate around the whole person.

Expanding Real-Time Care Coordination

Year 2 of the DBHDD Social Care Integration Initiative expands Care Event—or ADT—notifications. These near-real-time alerts can notify care teams when a patient enters, leaves or moves between care facilities, supporting proactive follow-up, helping close gaps across the care continuum and integrating information into existing EHR workflows.

The workflow turns data into action: a hospital generates an ADT event, the information is transmitted to GaHIN, the appropriate Community Service Board receives an alert, and outreach and care coordination can begin.

Making GeorgiaUnify Work Smarter

We're also enhancing GeorgiaUnify to make the platform easier and more efficient for the teams using it. Planned improvements include accessible training materials and guides, electronic signature capture for client consent, and a client registration function designed to improve data accuracy while reducing duplicate records and administrative burden.

Connecting Health + Housing

GaHIN is also working with the Georgia Department of Community Affairs (DCA) through a Corporation for Supportive Housing-funded initiative to advance bi-directional data exchange.

Today, housing-status information from DCA flows into GaHIN and is viewable in the clinical record. The future state expands the data connection so targeted, permissible information can flow back to authorized housing staff—helping verify program eligibility and coordinate services without repeatedly asking individuals experiencing housing instability.

WHAT'S NEW

GaHIN's work with DBHDD is also expanding into new populations and increasingly complex care environments.

Welcoming Assisted Outpatient Treatment Teams

Through the DBHDD Assisted Outpatient Treatment (AOT) Initiative, participating teams are able to use GeorgiaConnX to access clinical and behavioral health information to support informed treatment decisions and coordinated care planning.

GeorgiaUnify now includes additional AOT needs assessments and individualized care plans, while the Resource Directory helps the teams connect individuals with community-based services and track referrals through completion. The initiative also supports outcome monitoring and data-driven reporting.

Building Connections for Georgia's Children

GaHIN is also supporting MATCH—Multi-Agency Treatment for Children, a statewide initiative bringing together state agencies, healthcare providers and community partners to better coordinate care for children and youth with complex behavioral health needs.

Through GeorgiaConnX and GeorgiaUnify, participating teams can now access clinical information, streamline intake, conduct multi-domain assessments and coordinate community-based services. The goal is powerful: reduce service gaps, strengthen cross-agency coordination and improve access to the right care at the right time.

THE NETWORK IS MOVING FORWARD—TOGETHER.

Every new connection, workflow and partnership strengthens what GaHIN can deliver across Georgia.

To our members: THANK YOU for being part of the network and helping turn connected data into connected care.

And to organizations considering joining GaHIN: there has never been a more exciting time to connect!
