Attention Middletown Parents and Guardians,

Beginning November 22, 2021, our school district will be rolling out randomized PCR testing for asymptomatic students and staff members. In collaboration with the RI Department of Health, this process will consist of weekly PCR testing for a percentage of our student body along with staff members who have already consented to COVID testing. If you have not filled out the COVID testing consent and would like your child to be involved in randomized pool testing, please submit the form below. If you previously submitted this form, and have now decided that you would not like your child to participate in randomized pool testing, please resubmit the form and notify the school nurse of this change.

COVID Testing Consent Form:

Thank you and please contact me or your school nurse if you have any questions or concerns.

Outdoor recess
As the weather gets colder, please be sure to send students to school with jackets, hats, etc. for outdoor recess. Students will be going outdoors as much as possible.
A Note from Nurse Phillips
In order for medication to be dispensed to a student, the form below must be completed by a Parent/Guardian.

This form will be in lieu of a telephone call each time the student requires such medication while enrolled at Gaudet Middle School and Learning Academy. It is understood that parents will be notified if the request for medication becomes excessive.

Please signify your approval that the School Nurse may administer medication per School Physicians Standing Orders for minor complaints.

Administration of any other medication in school requires completion of a different form or a note from student's prescriber providing authorization.

Please contact the school nurse, Nina Phillips at 401-846-6395 or if you have any questions.

Community News!
The Middletown FOP Lodge 21 has set up a fundraiser for our FOP. We will be selling 30oz RTIC tumblers with the Middletown police patch engraved. Each tumbler is priced at $30.00 dollars. These will make great stocking stuffer gifts for the holidays.
We will take two forms of payment; Venmo (Timothy Beck "MiddletownFOP21"), and cash only. Please place your orders through this google form. If you wish to pay with cash, put it in an envelope with your name on it and either give it to K9 Officer Guerriero or drop it off at the Middletown Police Station.
