MPS COVID Testing
Attention Middletown Parents and Guardians,

Beginning November 22, 2021, our school district will be rolling out randomized PCR testing for asymptomatic students and staff members. In collaboration with the RI Department of Health, this process will consist of weekly PCR testing for a percentage of our student body along with staff members who have already consented to COVID testing. If you have not filled out the COVID testing consent and would like your child to be involved in randomized pool testing, please submit the form below. If you previously submitted this form, and have now decided that you would not like your child to participate in randomized pool testing, please resubmit the form and notify the school nurse of this change.

COVID Testing Consent Form:

Thank you and please contact me or your school nurse if you have any questions or concerns.

Outdoor recess
As the weather gets colder, please be sure to send students to school with jackets, hats, etc. for outdoor recess. Students will be going outdoors as much as possible.
A Note from Nurse Phillips
In order for medication to be dispensed to a student, the form below must be completed by a Parent/Guardian.

This form will be in lieu of a telephone call each time the student requires such medication while enrolled at Gaudet Middle School and Learning Academy. It is understood that parents will be notified if the request for medication becomes excessive.

Please signify your approval that the School Nurse may administer medication per School Physicians Standing Orders for minor complaints.

Administration of any other medication in school requires completion of a different form or a note from student's prescriber providing authorization.

Please contact the school nurse, Nina Phillips at 401-846-6395 or [email protected] if you have any questions.

Teachers will check in with students in Grades 4 & 5 at the beginning of the day and end of the day via Google Meets. If needed, office time or tutoring may be arranged with your child's principal.
One Time COVID Attestation Form
