New Article on AC Stewardship

We are thrilled to share the publication of a pivotal new article in JTH: Safe and Effective Anticoagulation Use: Case Studies in Anticoagulation Stewardship.


This article, led by AC Forum board member Dr. Jori May, dives into four patient case studies, underscoring how anticoagulation stewardship programs can close care gaps, ensure safe practices, and improve patient outcomes.

Energy and Impact at ASHP in NOLA


AC Forum’s activities at ASHP’s major pharmacy conference continue to expand and generate excitement.


A busy exhibit booth, well-attended learning session on Anticoagulation Stewardship, and networking among past, current, and future THM PGY2 RPDs and residents made for an action-packed and energizing week.


Our team is working to expand AC-related educational offerings at future ASHP conferences and plans are already underway for Vegas in December 2025.

TRAIN Pharmacy Residency Grant Funding

Spring 2025 Application Cycle Now Open!


The AC Forum is offering one 2-year grant totaling $120,000 to support the development of the ASHP-authorized PGY2 Pharmacy Residency program focused on thrombosis and hemostasis management. This funding assists institutions in establishing new programs and provides salary support for two consecutive residency years.


🔹Funding begins: July 2026

🔹Application deadline: Friday, February 14, 2025
