Although accountable care organizations (ACOs) have never been particularly popular among skilled nursing facilities, timing may be ripe to take on more risk to stay competitive in the market. ACO participation may help operators maintain referrals when so many potential residents are being diverted to home health care, hospice and other tracks along the continuum of care.

In order to stay healthy, patients need to receive proper testing and follow-up care from other providers after seeing their Primary Care Provider (PCP). However, disruption between PCPs and referred specialists often threatens that care quality. Healthcare analytics can provide insight for primary care providers and specialists to identify areas in which they can improve referrals, reduce out-of-network leakage, and pinpoint referral opportunities.
Telling a complete patient story requires capturing the social determinants that influence overall health. Evidence suggests that focusing on “upstream” interventions outside of traditional clinical care—such as housing, neighborhood conditions, and increased socioeconomic status—can lead to overall improvements in health and reduce health disparities and inequities.
