Dear ACT Center colleagues and friends,
We are so grateful to have spent the past year working alongside you to improve care for our patients and communities. In a year that brought many changes and challenges, our partners at Kaiser Permanente and in the broader community have stayed dedicated to learning what works best for patients and care teams — and then taking action to make improvements.
All of us at the ACT Center are proud to be part of this unique effort to transform health care and to see the positive impact of our partnerships — both through our work with community organizations and as part of Kaiser Permanente Washington’s Learning Health System (LHS) Program. Here are some highlights from the past year.
Training future LHS scientists in communities across Washington. With our partners in the Washington LHS E-STAR Center, our 2025 scholars launched 3 new quality improvement projects at the Veterans Affairs Puget Sound Health Care System and Kaiser Permanente Washington. The projects focus on improving social health, postnatal care, and flu vaccination rates across key patient populations. Other E-STAR scholars are working with NeighborCare and Community Health of Central Washington.
Creating pathways to better diabetes care in community clinics. In partnership with the Washington Association for Community Health, we are supporting the Pathways to Better Diabetes Care Collaborative in Federally Qualified Health Centers (FQHCs) across Washington state. The learning collaborative will convene 8 to 10 FQHCs to share best practices, learn from experts and peers, and test new evidence-based approaches to care.
Partnering with Kaiser Permanente Washington to improve care for our members. In 2025, our LHS Program worked with health system leaders to:
- Use advanced data analytics to identify members at high risk of hospital readmission and implement workflows and evidence-based interventions to prevent rehospitalization.

Develop and implement an Epic alert that indicates when children and adolescents may not be meeting healthy growth targets, helping care teams seize the opportunity for timely support and intervention.
