ACT team transitions to COPA/Partners In Recovery from Chicanos Por La Causa

This notice is to inform you that the Chicanos Por La Causa Assertive Community Treatment (ACT) team transitioned to COPA/Partners in Recovery’s Gateway clinic, 5222 E. Baseline Rd., Ste. 101, Gilbert, AZ 85234, effective July 1, 2020.

COPA/Partners in Recovery’s Gateway clinic is an integrated health home, or a one-stop shop that provides members both physical and behavioral health care.

We are excited to announce that the Gateway ACT team, since it is now embedded in an integrated health home, will serve as a PCP Partnership team. That means that members of the Gateway ACT team will have the option of receiving physical and behavioral health services at the same location as part of an integrated model of care.

ACT is a community-based service delivery model that is tailored for a member’s individualized needs and is available 24 hours a day, 7 days a week. ACT services are provided by small interdisciplinary team with specialties in peer support, housing, life skills, substance abuse treatment, nursing, counseling and psychiatry.
