Open Enrollment Elections!

All benefits-eligible employees must submit the following required forms by 12/2/2022 whether you are making changes to your coverage or not.

Follow the steps numbered below to complete the 2023 requirements
#1. Complete & Submit the 2023 Benefits Plan Election MS Form
Complete & Email requirements #2-6 securely,
(Protect your personal information by typing “secure:” in front of the subject line)

#2. Lincoln Basic Life Insurance Beneficiary Form *click on "Lincoln" for form*
Complete to designate your company-paid life insurance beneficiary,
*New Carrier, Lincoln
#3. Spousal Provision Form *click on "Spousal Provision" for form*
If you elect to cover a spouse on your medical plan, you are REQUIRED to complete the spousal provision annually or the spouse will
be removed from your plan Jan. 1, 2023.
#4. H.S.A Contribution Form *click on "H.S.A Contribution" for form*
Must Complete annually if you are enrolled in the High Deductible Health Plan
#5. F.S.A Contribution Form *click on "F.S.A Contribution" for form*
Optional: Complete if you would like to contribute to Flex Spending or Dependent Care F.S.A.
#6. Dependent/ New Hire Enrollment Form *click on "Dependent" for form*
Complete if you need to provide new or updated dependent information- for adding a spouse or a new child to your plan(s).
Provide marriage &/or birth certificates with securely.
#7. Voluntary Life Insurance Form
Request Application if interested in Voluntary Life Coverage for yourself or a family member.
#8. DIGWELL *click on logo to get to login
Submission of forms regarding DiGWELL Wellness Incentive Activities will not be accepted after 11/30/2022

Log in & be sure to invite your spouse, who is enrolled on your medical plan, so you both can participate in Establishing your Premium Reduction Incentive for 2023!
