Dear Vinay Kaza,

To confirm your eligibility for the MA Repay: Human Service and Home Health Workers Student Loan Repayment Program, you must submit the Annual Employment Verification Form by September 17, 2025. We are writing to notify you that you have not completed this required form. 

Failure to complete the form by December 16, 2025 will result in breach of contract. You will be responsible for paying a prorated portion of your total award amount to the Mass League within one year of your breach date. For more information, please refer to the No Response Termination Policy in the Policy Guide.

How to Complete the Annual Employment Verification 

1. Log in to your My MA Repay account here. On your account dashboard, click “Complete Verification”. 
2. Complete the form with your current employment information. 
3. Upload your most recent paystub. Please ensure the paystub includes your full name and organization name and is dated within 30 days. If you are a self-employed Family Child Care Provider, please upload your last-filed 1099.
