ANNUAL PAPERWORK

Hello AGG Families,

Paperwork is due July 28th by noon.

Everyone's favorite time of year has arrived... paperwork season. This year we have made the process easier by placing all of our policy handbooks online for families to agree to. Simply log into your Parent Portal and you will be prompted to agree to the policies for your athlete(s) program. Handbooks can be referenced at any time in your "Policies" section under "Account". Online policy agreements are considered part of the paperwork process this year and share the same due date listed above. While you're logged in to your account, please take a moment to check and update your contact information to ensure that we have your correct email addresses, phone numbers, and mailing address.

Hard copies will be handed out at practice starting Monday, July 17th, but you can download a digital copy with the link below. Extra forms will be available at the front desk if you misplace yours. All forms must be filled out in their entirety, and no spaces can be left blank. Please don’t write “Same As Last Year”.

Physicals must be completed and paperwork from your provider is to be turned in by September 30th. Per Sanford's direction, dates scheduled beyond September 30th will not be accepted. We want to make sure our athletic trainers are provided adequate information before shows and competitions begin.

2023-24 Paperwork

Please note that the Medical & Travel Release must be notarized for all athletes. We do have notaries on staff if you would like to set up an appointment to get your athlete paperwork notarized at AGG. Click the link below to schedule an appointment, spots are limited and will not exceed the listed times. Please remember to leave the second page of the Medical and Travel Release document blank until your notary appointment. An appointment must be made (no walk-ins, please) and will be offered at a first come, first served basis via the link below. Please remember to bring your drivers license. Otherwise, many local banks and UPS stores offer notary services.

All paperwork must be completed and submitted to the AGG Front Desk by noon, July 28th (this includes athletes who have joined the program this spring/summer). This date gives us adequate time to do what we need on the business end to ensure safety and security of all our athletes. Your athlete will not be allowed on the floor after the paperwork due date without all paperwork completed. Thank you for helping us make this process a smooth one.
