This is a reminder to all contracted providers for the above-mentioned lines of business to ensure provider staff who are responsible for the incident/accident/death (IAD) reporting process are appropriately registered and utilizing the AHCCCS QMS web portal application for submission of all IAD reporting. Providers are mandated to submit IAD reports utilizing this web portal application. Any IAD reports submitted from providers and received by Mercy Care Quality Management via email, mail and/or fax will NOT be accepted. These will be returned to the provider and requested for resubmission utilizing the AHCCCS QMS web portal application.
