APP Central Chicagoland Medical Director Announcement
Advocate Physician Partners is pleased to announce the appointment of Pranjal Shah, MD, MBA as Population Health Medical Director for Central Chicagoland.
 
Dr. Shah has over 15 years of primary care experience in clinical practice serving patients in Woodridge and Lemont. After finishing his medical school and residency in India, Dr. Shah moved to the Chicago area for his internal medicine residency. He has been working as an aligned physician with APP since 2008. He received his MBA at University of Notre Dame in 2016, where he graduated Magna Cum Laude and was the recipient of the John R. Malone Academic Achievement Award.

Dr. Shah currently serves as Good Samaritan PHO Medical Director. He has held several leadership positions within APP and at Good Samaritan Hospital. He is currently chair of the Quality Improvement Committee. He also serves as medical staff president-elect at Good Samaritan Hospital.

Dr. Shah has received multiple awards from APP for high CI performance over the last 10 years. Last year, he was nominated by Good Samaritan Hospital for the Advocate Aurora Health Leadership Summit Program - a leadership development program for select leaders, During the program, Dr. Shah and his team worked on a solution to improve health equity by community engagement.

In his new role, Dr. Shah will be responsible for clinical oversight of the Central Chicagoland PHOs, including Lutheran General Hospital and Illinois Masonic Hospital, to drive improvements in utilization, quality, and overall PHO performance. He will continue to serve as Good Samaritan PHO Medical Director. He will also lead the Population Health Medical Directors for Bundled Payment, Clinical Risk Adjustment, and Epic Connect.

We are very excited for Dr. Shah as he continues his leadership journey with the Enterprise Population Health team and wish him continued success.
