#3. Review RCA/PDSA Is your facility making any progress in overcoming your initial Root Cause for low VR referral and utilization rates? You will be asked to list two forms of education that you use for your QIA. Do you need new or additional materials?
#4. Patient Involvement in QAPI
Please discuss QIA progress and patient feedback in QAPI. Best Practice is having a patient present their activities/involvement in the QIA(s) at the beginning of the QAPI meeting - either in person or on the phone.
#5. Submit Monthly Status Report Online by 4/30/2019
Our work in April will be to build on the activities of the past months and work with your patients, family members, care partners, facility staff and others to improve vocational rehabilitation referrals and utilization rates rates.
Where you should be:
At this point (April), you should be formally tracking your patient education efforts, and trending their interest and feedback regarding Vocational Rehabilitation in patients 18-55 (QIA measure), and celebrating and encouraging education, hobbies, volunteerism, and employment with all of your patients. Remember: CROWNWeb is the data souce for the measure of this QIA. Is each CROWNWeb patient record up to date in the VR section of patient attributes? See our
CROWNWeb VR Update Guide.
Boundrilessness at the Network:We are working with other Network Programs, Independent Living Centers, and other stakeholders to identify resources and practices that can help you and your patients.
