We are almost half way through this year's QIA performance period of January-September where we work to meet the QIA goals:
Improve vocational rehabilitation referrals and Vocational Rehabilitation (VR) utilization rates.
Where you should be:
At this point (MAY), you should have already reviewed your VR education process (when and how do you screen your patients, and revisit their VR goals and needs), screened & referred your [18-54 (QIA Measure)] patients; and updated their CROWNWeb attributes.
For patients not in the 18-54 range, they should still be screened, and there are resources available for them too.

Set goals for QOL
Refer to VR/EN if and when patient is ready or requests

Access-VR to bring the needs of dialysis patients to the agency at the state level. We need to hear from YOU what you and your patients need to make VR services more accessible to your patients. Make sure you let us know on your monthly reporting so that we partner to make this work more accessible for ESRD patients.
