Advisory: Inspection of Fall Protection Lifelines

IHSA is reminding all employers, supervisors, and workers in the construction industry of the critical importance of inspecting fall protection equipment, especially lifelines (ropes), before each use.


There have been several recent incidents on jobsites where defective or damaged lifelines contributed to serious injuries. This advisory serves as a reminder that lifelines and all fall protection components must be thoroughly inspected prior to each use, and removed from service immediately if any damage or wear is detected.


Key reminders:

  • Ensure workers are trained to properly inspect and use fall protection systems, and to know when to remove equipment from service.
  • Inspect lifelines daily and before each use for cuts, abrasions, fraying, chemical damage, UV degradation, or any other signs of wear.
  • Follow the manufacturer’s guidelines for inspection, maintenance, and storage.
  • Do not use equipment if there is any doubt about its condition: “When in doubt, take it out.”
  • Use a rubber bumper or padding around any sharp edges to prevent cutting or fraying of the lifeline.
  • Document all inspections as part of your regular safety program.


Supervisors and employers:

  • Reinforce the importance of pre-use inspections during toolbox talks and safety meetings.
  • Make sure fall protection equipment is stored properly to avoid unnecessary wear and damage.
  • Replace worn or expired equipment immediately.
  • Conduct regular checks to verify that inspection procedures are being followed.


Fall protection is only as strong as its weakest link—and when lifelines are involved, the consequences of failure can be deadly. Stay vigilant. Stay safe.
