This month has been a magical month with all the winter themed events and the holiday season spirit all around.

The YMCA’s annual fundraising campaign makes it possible for the Y to provide opportunities for everyone in in our community to access vital community programs and resources that support youth development, healthy living and social responsibility.

You can help impact the lives of more people in your community by
donating or fundraising for the Y today.

Please join us to help scholarship a chilld today by donating to our Annual Campaign. All funds raised go back into the community to help families participate in our programs that would not be able to without the help of our campaign donations.

Childcare has a goal of $75,000 please consider helping us with your donation!

Helpful Resources and Documents
Afterschool Permission Form
Does your child participate in afterschool clubs, tutoring, etc? Before your child can attend these programs we will need you to fill out and turn in the Activity Permission Form. Please click the picture above or ask your Site Director for a copy.
Food Allergy and Medication Forms
If your child has a food allergy click the picture above to fill out our required forms. Please be sure to fill out the food allergy action plan and email to your Program Director. If your child will require any medication while onsite (EpiPen, Benadryl, inhalers, etc.), we will need the medication form along with the proper medication. If the medication does not have a prescription label you will need to get this form signed by the doctor.
Considerations/
Needs Intake Form
To help us better serve your family, if your child might require any additional services or needs any accommodations while at one of our afterschool programs, please fill out our Inclusions Intake Form and turn in to your Program Director.
