Reminders

Reporting Reminders

For PSA 13 agencies receiving Census funding from AgeOptions and for the other AAAs in Illinois receiving IDOA funding, thank you for your April reports! on the 5/12 call, Lucia reviewed these reporting reminders:
  • if you write numbers in the narrative section but do not report them in the grid, the activity is not counted. Make sure you add these in numbers and activities to the grid to count.Further details may be written out in the narrative
  • Be sure to include social media postings and reach. If you need help pulling this data, reach out to Lucia
  • Enter only numbers in the grid lines so the form can calculate your numbers properly
  • Columns M,N,and O are about Questionnaire Assistance. These columns are to capture how many people you helped fill out the Census, this number should be different from the total number of people reached. Questionnaire Assistance includes taking people through the online or paper form on the phone or connecting them to the hotline for the Census.

Click HERE for the reporting form. If you have any questions about the reporting form, please let us know.
