Capital Region Hospitals Draw Medicare Penalty for High Rates of Readmission

Saratoga Hospital formed a team two years ago to aggressively address readmissions. They found that frail elderly patients with chronic conditions were the group most frequently rehospitalized.
The hospital partnered with the Saratoga County Office for the Aging.

Women are still more likely to be caregivers, but the number of men caring for loved ones with Alzheimer's or dementia has soared from 19% to 40% in the past 15 years, according to the Alzheimer's Association. Read more.

State Submits Federal Waiver to Invest in Medicaid Redesign

Cuomo announced that New York has submitted an application for a waiver from the federal government that will allow the state to invest up to $10 billion in savings generated by the Medicaid Redesign Team (MRT) reforms to implement an action plan to transform the state's health care system.Read more.

 A new public campaign, The Conversation Project, was launched on August 15 "to have every person's end-of-life wishes expressed and respected." The campaign provides information and key resources, including a website. An ABC news article supports the Project, which will be featured on an ABC World News Tonight in the fall.
