Request for Applications: Community Care Hub

National Learning Community


The Administration for Community Living invites community-based organizations to apply for the 2023-2024 Community Care Hub National Learning Community by the November 3 deadline. Participants will gain access to peer support, technical assistance, and up-to-date information on resources, initiatives and collaborations aimed at furthering the capacity of community care hubs nationwide. The National Learning Community will launch in December 2023. Learn more and apply.

Register today for the Community Care Hub 101 Learning Series, hosted by the Aging and Disability Business Institute. The series of four webinars will occur on four Thursdays from mid-November through December 2023. LAACHA Shows How Collaboration Pays Off



The Aging and Disability Business Institute and the American Society on Aging (ASA) are collaborating on a series of articles focused on health equity for ASA’s Generations Now focused. The latest article, LA’s Alliance for Community Health and Aging Shows How Collaboration Pays Off (LAACHA), discusses the efforts of this alliance to bring together diverse multisector partners to address aging issues such as workforce development, innovations in technology, social isolation, loneliness, family caregiving and health equity. LAACHA’s membership includes government, community and academic organizations. Their members also include advocacy organizations such as Justice in Aging and AARP as well as philanthropic organizations like the LA Foundation on Aging. LAACHA strives to include more older adults as members and to support them as community leaders.

What CMS’s New GUIDE Model Means for

Dementia Patients, Caregivers and Providers



ATI Advisory hosted a webinar exploring the new Guiding an Improved Dementia Experience (GUIDE) Model that will reimburse providers managing dementia care for patients and their caregivers. This webinar included a panel discussion on policy changes and how this program could impact the care spectrum. Access the slides here.

CMS Releases Medicaid Transportation Guide


The Centers for Medicare & Medicaid Services released a new Medicaid Transportation Coverage Guide on federal requirements and state flexibilities. The guide also provides information on considerations for special populations, transportation under specific circumstances, provider and vehicle requirements and payment.

SNAP Participation Reduces Health Care Costs


New research from Benefits Data Trust (BDT), The Impact of Supplemental Nutrition Assistance Program (SNAP) Enrollment on Health and Cost Outcomes, shows one year of continuous participation in the Supplemental Nutrition Assistance Program (SNAP) lowers health care and pharmacy costs. BDT compared two groups—one group was enrolled in SNAP and the other was not. The data showed that the people participating in SNAP acquired health care costs that were 16 percent lower and pharmacy costs 21 percent lower.

CBOs seeking to contract with health care entities often need evidence about the effectiveness of their program to make a case for contracting. The Business Institute is seeking outcomes from successful contracts to add to its Evidence Bank. Does your CBO have outcomes data from your contracts that you are able to share? Let us know.
