Church Reporting Platform

for 2025-26 CC Forms is now OPEN!

Submissions deadlines and detailed instructions can be found on the platform itself. 

Let's get started:

• Enter your username and password;

Pastors, SPRC chairs, and Treasurers/Church Leaders may continue using the same login credentials as last year. Your Pastor has received username lists around noon today- August 13.

If you are a NEW pastor or SPRC chair and have no log in credentials, you will receive an email from the Mission Connect on Wednesday August 13; open your email and follow the prompts. If you don't receive one, first please check your spam/junk folder. If you still cannot find it in your spam/junk folder, please reach out to Lyn Moon at the District Office, LynMoon@unyumc.org.

Once you have entered the platform, click on “Church Reports” and then “Charge Conf Checklist" to see all the forms/reports that you need to complete.

Once you are there, see the top of the page to download the "2025-2026 Checklist" and "Help Document".

Notes for Forms/Reports

Most forms will need to be completed no later than October 1st, so pastors and church leaders are urged to begin accessing the platform. See "2025-2026 Checklist" for due dates. Also, Click the image below to see clearer version:

- HTML forms in dark blue boxes are now editable after saved/completed!

- Don't forget to complete "Local Church Leadership" online. There is "Help Document" for this form. So be sure that you read it first.

- You need to download, complete and bring THREE copies of Lay Minister's Annual Reports to the Charge Conference for approval and DS' signature.

- Complete forms/report online; make sure that you have all completed forms/reports uploaded or saved online; bring copies to the Charge Conference for DS signature and/or approval.

Compensation Form

You will also find a packet of information for your SPRC and church to set the pastor’s compensation for 2026. The completed compensation spreadsheets must be submitted through the platform. Below please click and download supporting documents:
