Five Reasons to Become a PACE Participant

PACE of Northeast Indiana is a Medicare and Medicaid funded program that combines both clinical and social care in one setting.


There are many reasons why PACE is an invaluable resource for caregivers and their care recipients. Here are just a few:

1) TRANSPORTATION


In an average month, a PACE Participant receives about 16 rides. As a part of the program, Participants are transported to and from the PACE Center in addition to any specialty appointments that aren't conducted in house.


2) SOCIALIZATION


Daily activities provide Participants with interaction amongst peers and our staff. This promotes both healthy relationship building and cognition. Depending on the activity of the day, fine motor skills are also used.


3) INDEPENDENCE


PACE helps people meet their health care needs in the community rather than in a nursing home or other care facility. 95% of current PACE Participants live in the community.


4) EXCEPTIONAL STAFF

5) PERSONALIZED CARE


The Interdisciplinary Team (IDTeam) spends time learning about each individual Participant to prepare a unique care plan specifically aimed at meeting that Participant's needs.

Family Caregiver Support Groups
