Your Voice Matters: Join All Care's Patient Focus Group | |

All Care Health Center invites patients to participate in quarterly Patient Focus Groups. These sessions offer patients a unique opportunity to share experiences directly with All Care’s leadership—what they value, what could be improved, and what services the community still needs. Leadership is also eager to hear thoughts from patients on the future of healthcare—concerns, hopes, and exciting upcoming changes.
Upcoming sessions include:.

All sessions will be held at All Care Health Center, and refreshments will be provided. No registration is required. Participating patients will receive a $25 Walmart gift card as a thank-you for their time and input.

Critical Substance Use Disorder Treatment Services are Launched at All Care | |

All Care Health Center is proud to announce that it has recently been licensed by the Iowa Department of Health and Human Services to provide substance use disorder (SUD) treatment services. This milestone marks a significant step in expanding access to integrated healthcare for the community, offering comprehensive support to individuals facing drug and alcohol use challenges.
The foundation for this program was laid by the Behavioral Health Expansion Grant from the Health Resources and Services Administration (HRSA), but it was through the vision and leadership of Amanda McCormick, Behavioral Health Director at All Care Health Center, that it became a reality. Her dedication and expertise have been the driving force behind launching this essential service, ensuring that individuals struggling with substance use have access to high-quality, compassionate care.
“Our community needs more access to treatment options for substance use disorders, and we are proud to be part of the solution," said McCormick.

All Care Health Center is pleased to welcome Jane Prine to the team, as an integral part of the SUD treatment program. Jane, a Licensed Alcohol and Drug Counselor, brings over two decades of experience in substance use disorder treatment and counseling, program leadership, and trauma-informed care, along with a passion for supporting individuals on their recovery journey.
"We are excited to welcome Jane Prine to our behavioral health team," said McCormick. "Our goal in adding Jane's talents is to help reduce the obstacles patients face when seeking treatment. It can be incredibly difficult to access care for substance use disorders, and delays are the last thing anyone should face when they're ready for help. Jane’s expertise and dedication will enhance our ability to provide timely, compassionate, and integrated care, supporting our mission to meet the growing needs of our community."
Jane brings a wealth of experience rooted in leadership roles, including Program Director and Director of Student Services at Uta Halee Academy and Assistant Program Director at Heartland Family Service. Her career reflects a profound commitment to addressing addiction, trauma, and behavioral health challenges. Her leadership has encompassed training law enforcement and community organizations, strategic program development, and advocacy for integrated care solutions. Jane’s extensive background includes facilitating substance abuse and family groups, overseeing therapeutic services, and managing comprehensive programs for vulnerable populations. Most recently, she served as a Licensed Alcohol and Drug Counselor at Rite of Passage – Uta Halee Academy, facilitating critical therapy groups and supporting program operations.

Dr. Glenn Hurst, who joined All Care in December, will play an additional key role in expanding All Care’s behavioral health services through the substance use disorder program. He will provide Medication-Assisted Treatment (MAT) to patients with opioid addiction. With a profound dedication to family medicine, these services are just part of Dr. Hurst's extensive background to All Care in areas such as weight loss, women’s healthcare, aging and geriatric health, rural healthcare, and various in-office procedures.
With All Care's new licensure and the unwavering commitment of our provider teams to patient-centered care, All Care Health Center is better equipped to address the growing need for substance use disorder treatment in Council Bluffs and surrounding areas. The program will offer a range of services, including individual therapy, group counseling, MAT, case management, and peer recovery support. By providing a fully integrated continuum of care, All Care aims to break down barriers to treatment and help patients achieve long-term recovery.

“This is a crucial addition to the services we provide at All Care,” said Joel Dougherty, Chief Executive Officer of All Care Health Center. “By integrating substance use disorder treatment with our existing medical, behavioral health, and dental services, we are strengthening whole-person care and increasing the opportunities for recovery and wellness within our community.”
McCormick stresses that the team at All Care is ready to support questions for problems related to alcohol or drugs, “We won’t judge; we’ll listen and give you the help you need.”

Achieving Better Health through Quality of Care

At All Care Health Center, increasing access to healthcare goes hand-in-hand with improving the quality of care for every patient. As a Federally Qualified Health Center (FQHC), we are committed to delivering high-quality healthcare to our community. One of the key ways we measure our impact is through the Uniform Data System (UDS), an annual report required by the Health Resources and Services Administration (HRSA) that tracks patient demographics, clinical quality, and financial performance. This data helps us evaluate our services, identify areas for improvement, and ensure accountability to our patients and funders.
In 2024, we made significant strides in patient care, improving performance on 13 out of 16 clinical quality measures. Notably, our efforts to support patients with chronic disease management. This led to remarkable progress in hypertension control, which increased from 53% to 66.5%, meaning more patients are keeping their blood pressure within a healthier range. Diabetes management also improved, with the percentage of patients experiencing poor blood sugar control dropping from 34.25% to 27.1%. Other gains include increased access to statin therapy (85.2% to 88.32%) and better mental health outcomes, with depression remission rising from 9.4% to 18.18%.
Preventive care saw meaningful improvements. To detect cancer when it's most treatable, cancer screenings rose across the board with colorectal cancer screening increasing from 31% to 38.54%. HIV screening showed a significant jump from 33% to 50.55%, allowing for earlier detection and access to life-saving treatments. Significantly more adult patients have a healthy BMI or a follow-up plan to address a low or high BMI, increasing from 43.5% to 69.93%; and 3% more patients are nonsmoking or receiving support to quit smoking. Additionally, preventive dental sealants for children increased from 14.29% to 66.67%, providing critical protection against cavities. These improvements reflect our team’s dedication to increased patient engagement and follow-up and enhancements to our systems, including electronic health record processes, improved workflows, and focused monitoring of progress throughout the year.
Looking ahead, All Care is committed to ongoing quality of care improvements through enhanced data and monitoring and continued workflow advancements. All Care will further strengthen preventative and chronic disease management by expanding integrated care and increasing case management, while continuing to improve financial assistance programs to ensure patients can access the care they need. These achievements are a testament to our incredible staff's hard work and collaboration—because quality patient care is truly a team effort.

Known for her unwavering dedication and compassionate approach, Maria strives to make teams highly efficient units focused on exceptional patient care. Maria’s extensive experience and expertise are invaluable assets. She comes to All Care following thirteen years of growth at One World Community Health Center, starting as a Family Practice Nurse, and elevating her skills through roles involving clinical services, triage, education, and quality, before completing three years as an Associate Nursing Director. In recent years, Maria furthered her education with a Master's of Science in nursing administration
Maria is not just a skilled nursing professional but also a supportive mentor who prioritizes listening to her team. Maria’s passion for nursing is truly inspiring. She motivates everyone around her to strive for excellence and fosters a culture of teamwork where each member is empowered to make a significant impact on the lives of our patients.
Maria is a familiar face in the community and shares her leadership expertise in many roles. She serves as a board member of the Council Bluffs Schools Foundation and the Omaha Cancer Society. She is also currently enrolled in the Women of Color Leadership program through the Iowa West Foundation. These are just a few examples of her broad community involvement that reflect her dedication to supporting local initiatives and making a positive impact.

Grube Heck Foundation of Chicago

All Care is excited to be the first community partner of the Grube Heck Foundation (GH Foundation) in its efforts to raise awareness of gastro health. The GH Foundation understands that discussions about digestive issues can be difficult, but it also knows these conversations are too important to avoid. Founders Kristin Lynn Grube and Dacia Heck, both personally affected by colorectal cancer, have become fierce advocates working to ease the path for others.
When Kendra Shirkey, Care Management Supervisor, met Dacia Heck of the GH Foundation at the American Cancer Society National Colorectal Cancer Roundtable, she was eager to share All Care’s commitment to increasing colorectal cancer awareness. The GH Foundation quickly decided to partner with All Care and will adopt some of our innovative ideas into its own outreach efforts.
This year, All Care is expanding education beyond our health center by reaching public housing locations and all of our clinic sites. The GH Foundation is supporting All Care in encouraging patients to complete colorectal cancer screenings—whether through a FIT Test, Cologuard, or colonoscopy. Patients who complete a screening during March will be entered to win a small gift from the GH Foundation.

All Care’s colorectal cancer awareness committee, led by Shirkey, has organized various events to engage the community. Art students from Thomas Jefferson have returned to All Care, along with their principal, Mr. Naughton, to use their creativity and paint the windows at All Care to draw attention to the cause. Last year, the project left a lasting impression. Thomas Jefferson’s art teacher, Laurie Bancroft, was already brainstorming ideas for 2025 before the paint had even dried.

And once again, keep an eye out for the blue glow of the Hero’s Fountain, located between Pearl and Main Streets at West Broadway, just south of the First Responders Memorial Plaza, as it shines brightly to raise awareness for colorectal cancer.

Making a Difference in Patient Lives: A Story of Care | |

Every patient's health journey at All Care Health Center is an opportunity to give meaningful care. Jazmine, a Health Navigator at All Care, recently shared an inspiring example of how her role, in partnership with our providers, helps patients overcome challenges and achieve stability in their care plan.
Ben, a medical provider at All Care, was eager to help Isabel* better manage her diabetes by using a continuous glucose monitor at home. Isabel’s family had attempted to navigate the process independently but faced obstacles. When Jazmine reached out to Isabel and her family to fully understand the situation, she quickly realized that language barriers may have been preventing them from successfully obtaining the device.
With the help of All Care’s MARTTI interpreter services, Jazmine was able to communicate effectively with Isabel’s family and gather the necessary information. Shortly thereafter, the order for the continuous glucose monitor was completed, and the device was shipped directly to Isabel’s home. Grateful for the support, Isabel shared how easy the monitor was to use. She expressed her appreciation for All Care’s assistance, knowing she could turn to us for any future concerns.
