Social Prescribing Day is a global celebration of how communities enhance wellbeing and transform health systems through social prescribing. The Canadian Institute for Social Prescribing (CISP) is hosting a week to showcase the power of this innovative approach to healthcare and explore how it is making a difference across Canada.


They are hosting a series of virtual events with thought leaders and experts who will dive deep into the future of social prescribing in Canada. These informative sessions will explore key topics and innovations shaping the movement. These include From Evidence to Action: The Role of Social Prescribing in Health Systems, Building Integrated Care: Diverse Approaches to Social Prescribing in Canada and Canadian Social Prescribing Exchange.

For over 50 years, the organizations that make up the Alliance for Healthier Communities have been delivering comprehensive primary health care that exemplifies the kind of change we envision for the whole health system, by providing care for people and communities as described in our Model of Health and Wellbeing (MHWB). Alliance members have created connected health neighbourhoods by reaching beyond their doors, creating partnerships, and integrating health and social care. In this webinar, we will share the Alliance's vision of a system of primary health care based on the MHWB - a vision that, if implemented, has the potential to vastly improve the health and wellbeing of people across Ontario.


This evidence-informed vision describes a health neighbourhood model consisting of:


  • Interprofessional, collaborative primary health care
  • Community governance and engagement
  • Population-based approach to health
  • A strong data foundation

Launch of French Social Prescribing Online Learning Webinar

Please join us in launching the French version of our health equity-focused social prescribing online learning modules. Our health equity-focused, online Social Prescribing training modules are designed for healthcare teams interested in implementing or improving their social prescribing process. These self-directed modules include Getting Buy-in from Interdisciplinary Teams, Co-designing a Process Map, Building an Asset Map, Meeting with Clients, and Using the EMR to Track the Client's Journey, among others. Learn everything from key terms and concepts to practical tips on creating helpful resources tailored to your workplace and working with clients. The modules can be taken individually or as a whole course.
