Our new quality goal to improve care for individuals living with dementia focuses on knowing the person, building on strengths, filling days with positive moments, and proactively addressing unmet needs. Our new goal includes appropriate use and reduction of psychotropic medication as part of whole-person care.

Coming soon, the Tracking Tool has been expanded to encompass all psychotropic medications with form-based data entry, GDR reminders, tracking to support positive approach to meeting unmet needs, and plenty of charts and graphs to inform your quality improvement efforts.
YOUR PARTICIPATION MATTERS
CMS, the CDC, and the Quality Innovation Network Quality Improvement Organizations encourage nursing homes to continue to track and report C. difficile data to NHSN. These data inform national work on patient safety and are valuable for individual nursing home quality improvement projects, including C. difficile prevention and antibiotic stewardship.
