St. Anne's Mead presents the Next Step program, designed for senior loved ones being discharged from a Skilled Nursing / Rehab Facility or Hospital Stay.

SOUTHFIELD, MICHIGAN -- St. Anne's Mead announces the Next Step program for senior loved ones being discharged from a Skilled Nursing / Rehab facility or Hospital stay. "We receive many inquiries from both discharge planners and anxious family members seeking help for patients being discharged that really need continuing care," said Robin Harris, Director of Admissions.

The Next Step program meets the needs of those patients with customized assisted living. "It is not practical for many seniors to move back home after a hospital stay and pay for a caregiver around the clock," said Amanda Newport, Director of Nursing and Assisted Living. "The Next Step program provides around the clock support while allowing them to continue physical and occupational therapy and participate in social activities, right here at St. Anne's Mead," she said.

Families are encouraged to be part of their loved one's care, and, to give them the opportunity to try out St. Anne's Mead, a Respite "Short Stay" program is offered from one week up to two months. "Families can feel confident their loved ones are continuing on the path of recovery, while receiving the care and support they need," added Harris.
