Upcoming Event: Annual Health Screening

On-site and off-site health screenings will be available to employees and spouses* enrolled in the QCR Holdings, Inc. Medical Plan

*Important note: Rock Valley Health would like to request a separate email address per screening participant - Please reply to this email with your spouse's email address, if applicable. Thank you for your help with this matter.

Schedule your on-site Biometric Screening:

• Date range: August 15 - September 28

• All appointment times begin at 7am (exception - Saturday, September 16th).

• If your spouse is covered by the health plan, please ensure that they are aware of the upcoming biometric screening and that they schedule a time for their on-site screening appointment or receive the off-site form to have their doctor complete and fax back to Rock Valley Health.

• Once your appointment is officially booked, you will have the option to click on the calendar icon (e.x., Outlook, Google Calendar, etc...) toward the bottom of your confirmation page to conveniently sync your appointment to your online calendar.

Participants who will be visiting their primary care physician to complete their health screening, please click on the box below and follow instructions listed.

Valid Dates: January 1, 2023- December 31, 2023

Deadline: Must submit off-site form by December 31st.

*If you are pregnant please reply to this email so Rock Valley Health can make sure you receive an exemption from the wellness program. If you have a spouse on the plan they will still need to participate*
