HCCI seeks applicants for the second cohort of its Illinois House Call Project. Our second cohort will include highly qualified Illinois medical practices, including independent entities, those affiliated with hospital systems, and hospice and palliative care programs looking to add or expand their home-based primary care services.

Each selected practice will designate up to three “Home Centered Care Champions” (providers and practice professionals) to lead their work. The new goal of the ILHCP will be to collectively enroll 6,000 new homebound or home-limited people in their homes from 2022 to 2026. HCCI will provide the Champions with expertise, guidance, and comprehensive resources, including education, consulting, tools, market analysis, marketing/PR training, and the opportunity to receive capacity-building funding.

Complete this ILHCP Cohort 2 Application by 11:59 PM CST on March 18. Selected practices will receive notification by mid-April.

Join HCCI at “Actionable Market Intelligence for Home Health,” a free use-case Demo and Discussion on Tuesday, March 19, at 3pm CT. During the session, participants will learn about available home health market intelligence, including performance and referral relationship information. Providers and practices can use the data to build stronger partnerships with those who refer patients to them by showing their competitive strengths and key differentiators.

Join our in-person event in the Crossroads of Care series for Illinois hospice and palliative care programs, Crossroads of Care: Contracting with Payers to Demonstrate the Value of Home-Centered Care, on Thursday, March 7, 4-6 pm CT, in Schaumburg, IL. Our expert faculty will discuss the various payer types relevant to home-based primary care and home-based palliative care, identify key individuals to connect with when negotiating contracts, and how to demonstrate the value of your home-centered care services through the presentation of quality-of-care metrics, utilization and outcomes data, and compelling patient stories. A light supper will be provided.
