Due to significant interest, the application deadline for the second cohort of the Illinois House Call Project has been extended to Friday, May 3. During this time, HCCI continues to invite passionate Illinois providers to apply for selection — including independent entities, those affiliated with hospital systems, and hospice and palliative care programs looking to add or expand their home-based primary care services.

• Get a Practice “Checkup”: Take part in a confidential evaluation of your practice by HCCI, including billing and coding, geographic scheduling, staffing, and more. Then we will show you how to use tailored market intelligence from our Confer Analytics™ platform to pinpoint opportunities for growth.

Karly Denes, HCCI’s Outreach and Engagement Specialist, will also support the selected practices. “I’ll hold a practice’s ‘hand’ throughout the whole process,” Denes said. “We're going to offer many types of support. If, for example, you need assistance setting up a staffing model for offering house calls or billing for a specific service, then I’ll collaborate with you and find resources for whatever challenge you're facing.”
“This is something that's so personal to me and the people who work for HCCI,” she added. “We have personal experiences with family, friends, and community members needing this kind of support, so we want to emphasize how impactful home-based primary care is.”
